Request a Statement of Widow's, Widower's or Surviving Civil Partner's (Contributory) Pension

You can request a statement of the Widow's, Widower's or Surviving Civil Partner's (Contributory) Pension payments you have received online and the statement will be sent out to you in the post.

You do not need to request a statement of the Widow's, Widower's or Surviving Civil Partner's (Contributory) Pension payments received from 2011 onwards for Revenue/tax purposes as the Revenue Commissioners already hold this information. Where a Revenue/tax statement is required for years prior to 2011 or if you require a statement of your payments for any other purpose, for any year, you should complete the online form.
